Par-Q, Medical History & Lifestyle Questionnaire
These questionnaires range in types of questions from basic get-to-know you questions to questions about past injuries and illnesses to questions about lifestyle habits. They’re designed to determine whether you should see a doctor prior to increasing your activity level.

Expectations & Goal Setting
This is where we will deliver our expectations of you and ask you to fill out short and long term fitness goals that adhere to the S.M.A.R.T. acronym. Specific, measurable, attainable, realistic and time oriented.

Strength & Conditioning Evaluations
This will be the actual strength and cardio training through which our personal trainers will show you how to properly use equipment, use safe technique and test your current physical status.

Why do these assessments?
In order to create a completely individualized training regimen to help you reach your goals we need
to know as much as possible about your current physical status as well as your current eating and sleeping habits. This will allow us to create a personalized program through which we can help you reach your goals. Just remember that we can only control so much and in order for you to reach your goals you must be
compliant with attendance and effort along with making reasonable nutritional and recovery changes.
